ACORD 25

The standard certificate of liability insurance form used to verify a subcontractor's insurance coverage.

ACORD 25 is the industry-standard Certificate of Liability Insurance form published by the Association for Cooperative Operations Research and Development. It provides a snapshot of an insured party's coverage, including general liability, auto liability, umbrella/excess, and workers' compensation policies.

The form includes critical fields such as policy numbers, effective dates, coverage limits, additional insured endorsements, and the description of operations. General contractors use ACORD 25 forms to verify that subcontractors carry adequate insurance before allowing them on the jobsite.

Important: An ACORD 25 is a certificate of insurance, not a policy. It serves as evidence of coverage but does not amend, extend, or alter the terms of the actual insurance policy.
